It's most likely a medical electrode like those used for EKG/EEG.
What makes it unusual is that the wire looks either very corroded or
like it's made from tightly braided horse hair.

What is?
Richard Morgan had a box of those things in his house, we see them
when Rachel is watching SM00015. Pretty neat that Rachel experienced
not only mental manifestations of Samara's life but physical as
well.

Yes, except that everywhere you see the electrodes in the movie,
the wires are white. The one that comes out of Rachel's throat is
black. I thought it was Samara's hair and not horse hair, but it's
anybody's guess.
Also, I think it was part of the dream, not
a physical manifestation.

I too, thought that what Rachel pulls out of her mouth was Samaras
hair attatched to one of the medical electrodes. But in the cursed
video, I thought what the guy has coming out of his mouth looked
more like an intestine than anything else to me.

I am really going to have to say it's an electrode. THe one in the
cursed video is white, and looks like Samara is pulling it out of
herself, the the well with water all around. The one Rachel pukes up
is braided, but still with the large electrode at the end. Remember
when she watches SM0015, the doctor places a long cord to her foot,
and it is attached to the wall? Perhaps this is the cord.

As for it being big, remember this is 1978, so if it is an
electrode, it would be larger perhaps than the ones we use now?
Also, the medical facility she was at didn't look very up to date,
possibly using these older wider cords.
